I (James) stumbled across this hymn online. The lyrics are well-written by Francis Ridley Havergal and it's the same tune by Samuel Sebastian Wesley as has been frequently used for "The Church's One Foundation." It would work well for a New Year's Eve service or a church service in January. Lyrics below:
